JobDescription Summary
The speech–language pathologist is responsible forimplementation of standards of care for speech language pathology services andfor adherence to all conditions in the Service Agreement.
Report to: Director of Patient Services
Job Duties (include, but are not limited to the following):
- Assists the physician in evaluating thepatient’s level of function. Assesses home environment and identifies equipmentneeds related to speech/language function.
- Identifies functional speech deficits andestablishes goals and plan of care to improve patient’s function, revising asnecessary.
- Performs and completes admissions,recertifications, resumption-of-care, and discharge Comprehensive OASISassessments and associated paperwork in accordance to agency policy.
- Instructs patient and family members, as well asother health team members, in methods of assisting the patient in improvingcommunication skills.
- Recommends communication devices/aids asindicated.
- Periodically re-evaluates and upgrades thepatient’s program to maximize functional improvement. Records and reports tothe patient’s physician the patient’s progress in treatment and any changes inthe patient’s condition and plan of care.
- Confers with the family, referring physician,and other agency professional staff to obtain information to promotecoordinated, efficient, and quality patient care.
- Performs and submits required documentation ofpatient’s evaluations, plan of care, clinical notes, and progress reports,summaries along with changes in orders. All patient visit notes must be turnedin the next scheduled work day, not to exceed forty-eight (48) hours.
- Maintains necessary statistical and billingdata.
- Keeps abreast of changes in speech pathologypractice and theory, contributes to the field through research, presentations,and in-services.
- Keeps his/her immediate supervisor informed ofproblem areas and makes suggestions for improvement.
- Attends and contributes to in-services, clinicalrecords reviews, meetings required to ensure coordinated and comprehensiveplans of care for the patients of the agency.
- Complies with Homecare Agency Compliance Plan.
- Complies with Homecare Agency’s InfectionControl Policies.
- Complies with Homecare Agency’s HIPAA Policies.

PositionQualifications
- Possesses a degree that includes an internship from an approved Master’s program in Speech-Language Pathology, or as required by applicable law and/or regulation.
- Meets the education and experience requirements for a Certificate of clinical Competence in Speech-Language Pathology, granted by the American Speech-Language-Hearing Association (ASHA).
- Licensed in the State of practice by the State Board of Healing Arts to practice as a Speech Pathologist.
- Is certified to practice Speech-Language Pathology.
- A minimum of one year’s clinical experience in Speech-Language Pathology.
- Demonstrates good verbal and written communication, and organizational skills.
- Possesses and maintains current CPR certification.
