Job summary

Evolve Therapy Services in Lamberton, MN is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility. This position offers Full Time, Part Time, or PRN options to fit your schedule.

You will plan, organize, and direct Speech Language Pathology services, ensure CMS documentation in NetHealth EMR, and coordinate FEES/MBS protocols with site programs to promote resident care.

Qualifications

• Master's degree in Speech Language Pathology from an accredited program.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice with ongoing CE.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target list, short-term, and long-term goals.
• Instruct patient, family, and staff to ensure carryover of therapy in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols with site program coordination.
• Participate in screening protocols to ensure needs of residents are met timely and with clinical excellence.

Education

Active SLP license in practice states

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
