Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Worthington, MN. The role supports residents with comprehensive evaluation, treatment planning, and therapy delivery.

Full Time, Part Time, or PRN options available. The clinician will ensure CMS-compliant documentation in NetHealth EMR and coordinate FEES/MBS protocols as needed.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• CCC certification required for practicing clinicians.
• Active state licensure in at least one practicing state.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide high-quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and progress.
• Develop individualized treatment plans with targets and goals.
• Instruct patients, families, and staff in speech-language procedures for daily carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to determine residents' needs and ensure timely care.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
