Job summary

Evolve Therapy Services, LLC, a therapy management organization, seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility partner in Tiffin, OH.

Full Time or PRN position with responsibilities including speech therapy services, data collection for treatment planning, development of goals, and education for patient, family, and staff. CMS documentation in NetHealth EMR and oversight of FEES/MBS protocols are required.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC credential.
• Active SLP license in practicing state(s).
• Commitment to ongoing continuing education.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with short-term and long-term goals.
• Educate patient, family, and staff for carryover of techniques.
• CMS documentation in NetHealth EMR per CMS expectations.
• Oversight of FEES/MBS protocols with site coordination.
• Participate in resident screening to determine needs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
