Job summary

Evolve Therapy Services, LLC in Tiffin, OH is seeking a licensed Speech-Language Pathologist for an in-house Skilled Nursing Facility to deliver high-quality therapy services. You will evaluate residents, develop treatment plans, and coordinate with the care team to meet CMS requirements.

Ideal candidates hold a Master's in Speech-Language Pathology, CCC, and an active state license, with experience in FEES/MBS protocols and CMS documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech-Language Pathology or equivalent
• CCC certification required
• Active SLP license in state(s) of practice and ongoing licensure maintenance
• Commitment to ongoing continuing education and professional development

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate
• Evaluate and obtain data for treatment planning and implementation
• Develop treatment plans with target lists and short-/long-term goals
• Instruct patient, family, and staff in speech-language procedures for carryover
• Complete CMS-compliant documentation in NetHealth EMR
• Oversee FEES/MBS protocols and site program coordination
• Participate in resident screening protocols and ensure timely care

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
