Qualifications

Responsibilities

• Assess speech, language, cognitive-communication, and swallowing abilities to determine residents' needs.
• Create individualized plans of care with functional, measurable treatment goals.
• Deliver therapy addressing communication, memory, attention, and problem-solving during daily tasks.
• Provide dysphagia treatment and coordinate swallowing recommendations with nursing and dietary teams.
• Involve residents, families, and caregivers in progress strategies between sessions.
• Maintain clinical documentation, monitor outcomes, and support care transitions and discharge planning.
• CFY supervision and guidance will be provided during fellowship.

Ultimate Therapy is hiring a full-time Speech-Language Pathologist at Autumn Lake at Cherry Hill, with Clinical Fellowship Year (CFY) opportunities available. Provide rehabilitation that helps skilled nursing residents express their needs, stay engaged in daily activities, and manage swallowing difficulties.

What You'll Do

• Assess speech, language, cognitive-communication, and swallowing abilities to determine each resident's needs.
• Create individualized plans of care with functional, measurable treatment goals.
• Deliver therapy addressing communication, memory, attention, and problem-solving during everyday tasks.
• Provide dysphagia treatment and coordinate swallowing recommendations with nursing and dietary colleagues.
• Involve residents, families, and caregivers in learning strategies that support progress between sessions.
• Maintain clinical documentation, monitor outcomes, and contribute to care transitions and discharge planning.
• Clinical Fellows will develop their skills with required supervision, feedback, and guidance.
