Springfield Center for Living & Rehabilitation is seeking a Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team in Springfield, Vermont. This role focuses on residents with speech, language, cognitive-communication, and swallowing needs.

The ideal candidate holds a Master's in SLP, Vermont license, CCC-SLP, and provides evidence-based interventions within an interdisciplinary team, documenting in Net Health and PCC, and participating in care planning

Qualifications

• Master's degree in Speech-Language Pathology.
• Current Vermont SLP license and CCC-SLP certification.
• Experience in SNF/LTC settings preferred.

Responsibilities

• Evaluate and implement individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.
• Provide evidence-based interventions to maximize outcomes and quality of life.
• Collaborate with physicians, nursing, rehab staff, residents, and families.
• Complete timely documentation in Net Health and PCC.
• Participate in care planning meetings and interdisciplinary discussions.
• Ensure compliance with state, federal, and facility regulations.

Qualifications

• Master's Degree in Speech-Language Pathology
• Current Vermont Speech-Language Pathology license
• Certificate of Clinical Competence (CCC-SLP) required
• Skilled nursing, rehabilitation, or long-term care experience preferred
