Job summary

Springfield Center for Living & Rehabilitation in Vermont is seeking a Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team in a skilled nursing and long-term care setting.

In this role you will evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders, and you will document outcomes in Net Health and PCC. Collaboration with an interdisciplinary team is essential.

Qualifications

• Master's in Speech-Language Pathology and valid Vermont SLP license.
• CCC-SLP certification required.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational abilities.
• Commitment to resident-centered care and quality outcomes.

Responsibilities

• Evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.
• Provide evidence-based interventions to maximize functional outcomes and quality of life.
• Collaborate with physicians, nursing, rehab staff, residents and families.
• Complete timely documentation in Net Health and PCC.

• Complete timely and accurate documentation in Net Health and PCC

• Participate in care planning meetings and interdisciplinary team discussions

• Ensure compliance with all state, federal, and facility regulations

Qualifications

• Master's Degree in Speech-Language Pathology

• Current Vermont Speech-Language Pathology license

• Certificate of Clinical Competence (CCC-SLP) required

• Skilled nursing, rehabilitation, or long-term care experience preferred
