Job summary

The Grand Rehabilitation & Nursing at Barnwell in Valatie, NY is seeking a Speech Language Pathologist (SLP) for full-time or per diem engagement. The role focuses on evaluation and treatment of residents with speech, language, cognitive, voice, and swallowing disorders in a skilled nursing setting.

You will collaborate with nursing, therapy, and medical teams, develop individualized plans, document progress, and participate in care plan meetings.

Qualifications

• Master’s degree in Speech-Language Pathology.
• Current NYS license as a Speech Language Pathologist.
• ASHA CCC-SLP preferred.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide therapy to improve communication, cognition, and safe swallowing
• Document evaluations, progress notes, and discharge summaries accurately
• Ensure compliance with state, federal, and facility documentation standards
• Communicate treatment plans and progress with nursing, rehab, and medical staff
• Participate in care plan meetings and interdisciplinary discussions
• Educate residents, families, and staff as appropriate

Position Summary

Primary Responsibilities

Evaluation & Treatment

• Evaluate residents for speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide therapy to improve communication, cognition, and safe swallowing

Documentation & Compliance

• Document evaluations, progress notes, and discharge summaries accurately
• Ensure compliance with state, federal, and facility documentation standards

Interdisciplinary Collaboration

• Communicate treatment plans and progress with nursing, rehab, and medical staff
• Participate in care plan meetings and interdisciplinary discussions
• Educate residents, families, and staff as appropriate
