Job summary

Bradford Hills Nursing & Rehabilitation Center is seeking a Full-Time Speech-Language Pathologist to join our in-house rehabilitation team. CFY candidates are encouraged to apply and will work closely with an interdisciplinary team to deliver therapy that improves residents' communication and quality of life.

You'll evaluate residents, create individualized treatment plans, and document progress using Net Health and PCC.

Qualifications

• Master's Degree in Speech-Language Pathology required.
• Current Pennsylvania SLP license or eligibility for licensure.
• CFY candidates are encouraged to apply.
• Strong communication, organizational, and interpersonal skills.
• Passion for serving geriatric residents in SNF/Long-Term Care.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide treatment for speech, language, cognitive-communication, and swallowing disorders.
• Collaborate with nursing, therapy, dietary, and medical teams to maximize outcomes.
• Educate residents, families, and caregivers throughout rehabilitation.
• Maintain accurate, timely documentation using Net Health and PCC.
• Help residents achieve their highest level of communication, cognition, safety, and independence.
