Medical Facilities Of America, Inc. in Delaware is seeking a full-time Speech Pathologist to provide direct patient care in a long-term care setting. The role includes assessments, treatment planning, goal setting, documentation, and collaboration with care teams.

The position offers an hourly pay range of $42-$52 for FT, with a $10K retention bonus. PRN and PT positions are also available with higher hourly rates; state licensure required.

Qualifikationen

• Must have graduated from an accredited Speech Therapist college or university.
• Must hold a valid professional license and MD state registration.
• Experience with long-term care and/or geriatric care preferred.

Aufgaben

• Assessment of referred patients that require rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition.
• Educate patients and families in an appropriate therapy method.
• Perform evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Perform all other duties assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPAA compliance and residents' rights.

FT Position offers a $10K Retention Bonus.

Pay Range is dependent on Status (FT/PT/PRN) and years of experience.

FT Pay Range is $42-$52 per hour

PT Pay Range is $45-$53 per hour

PRN Pay Range is $55-$60 per hour

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
