JOB PURPOSE: The Speech-Language Pathologist provides therapy services to physically ill, disabled, mentally ill/developmentally disabled patients with speech, language, swallowing, and hearing disorders in the home setting. Ages: Pediatric to Geriatric. Work includes responsibility for the assessment, planning, implementation, and evaluation of patient’s speech-language pathology therapy plan of treatment. Salary listed is for full time status.

QUALIFICATIONS:
  1. Minimum of one year practicing as a Speech Language Pathologist.
  2. Must possess a Master’s Degree in Speech-Language Pathology.
  3. Possess and maintain a current license as a Speech Language Pathologist from the Maryland State Board of Speech Language Pathology Examiners.
RESPONSIBILITIES:
  1. Conducts a complete speech/hearing/language/swallowing evaluation in order to determine appropriate treatment in accordance with the functional level of the patient.
  2. Obtains a complete history through interview of patient/family/others.
  3. Plans, develops, implements, and evaluates speech/language therapy treatment for patients in accordance with the physician’s plan of care and the patient’s abilities/limitations.
  4. Completes a plan of care that is thorough with measurable short and long-term goals.
  5. Conducts speech/language/hearing/swallowing treatments.
  6. Teaches home speech/language therapy programs to patients and/or their families and assists them in planning/implementing equipment needs for home use.
  7. Observes and records the reactions and progress of patients receiving therapy.
  8. Regularly reevaluates the rehabilitation needs of the client and modifies speech/language therapy programs based upon those needs.
  9. Observes signs and symptoms and reports to the physician and/or other appropriate health care professionals as often as needed or as changes in the patient’s condition requires.
  10. Completes visit notes, orders for therapy, progress notes, discharge summaries, and other documentation as needed to comply with Licensure, Certification, Accreditation requirements, and agency policies.
  11. Orders supplies and equipment for patients.
  12. Attends case conferences in order to assure interdisciplinary communication regarding patient status.
  13. Participates in QI activities.
  14. Performs duties in compliance with Speech-Language Pathology Standards of Practice.
  15. Performs additional activities as assigned.
  16. Attends staff meetings and in-services as required.
Requirements
SPECIAL REQUIREMENTS:
  1. Possess and maintain documentation of freedom of TB in a communicable form.
  2. Possess and maintain documentation of annual influenza vaccination.
  3. Mandatory Competencies as defined by the agency Education Plan
  4. Maintain current CPR certification.
  5. Maintain current driver’s license.
  6. Maintain current auto insurance of at least $100,000 total liability insurance per person and $200,000 total liability insurance per accident.

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling and stooping all day. The employee must frequently lift or move items weighing over 20 pounds. This position requires the ability to transfer, lift, move, and turn patients to and from bed, wheelchair, toilet, tub, shower, and occasionally the floor.

WORKING CONDITIONS:

1. Work is performed primarily in the patient’s home.

2. Exposure Risk to Blood borne Pathogens: Category A - routine exposure to blood and body fluids. Occasional exposure to hazardous materials and infectious diseases.

3. Extensive travel is required, sometimes in hazardous weather conditions.

EMERGENCY RESPONSIBILITY:

Expected to respond to emergency situations involving the safety of the patient in their homes.

This includes the ability to assist with a possible evacuation of patient, or notify the proper agency/facility in case of any type of emergency situations such as calling an ambulance or the fire department.
