Job summary

21st Century Home Health Services in Palo Alto is seeking a licensed Speech Therapist/Language Pathologist to deliver in-home services across the Santa Clara County region. You will assess and treat communication, swallowing and cognitive disorders, working closely with case managers.

This role offers flexible schedules, autonomy, mileage reimbursement, and comprehensive benefits, with a strong emphasis on patient-centered care and collaboration within an interdisciplinary team.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and culture in health care.
• Home health care experience preferred.
• Experience with high acuity patients a plus.
• Experience working in an interdisciplinary team.
• Case management fundamentals desirable.

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team

To be a success as Speech Therapist/Language Pathologist (ST)

• Knowledge of state and federal homecare regulatory guidelines preferred.
