Job summary

Serenity-Therapy-Solutions-LL is seeking a per diem Speech-Language Pathologist (SLP) for Mohawk Meadows Nursing Home in Lafayette Township, NJ. The role offers compensation of $55–$65 per hour in a supportive clinical environment.

Responsibilities include evaluations, therapy planning, documentation, and collaboration with an interdisciplinary team to ensure high-quality patient care and adherence to regulations.

Qualifications

• NJ SLP license required.
• SNF experience preferred.
• Knowledge of dysphagia and cognitive rehab preferred.
• Strong communication and organizational skills.
• Experience with Rehab Optima is a plus.

• Complete timely documentation including daily notes, progress reports, evaluations, and discharge summaries.
• Treat residents with speech, language, cognitive, communication, and swallowing disorders.
• Work closely with interdisciplinary team members to ensure quality patient care.
• Educate patients, families, and staff on therapy goals, progress, and therapeutic techniques.
• Ensure compliance with facility protocols and state/federal regulations.
