Job summary

Trilogy Health Services in Kokomo, IN is seeking a Speech Language Pathologist to provide comprehensive assessment, treatment planning, and therapy in an interdisciplinary setting, with emphasis on communication and swallowing disorders.

You will evaluate patients, develop individualized plans, document progress, educate families, and collaborate with a care team. This role requires state licensure and CCC-SLP preferred; travel is minimal.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Licensure as a Speech-Language Pathologist in state of practice required.
• CCC-SLP preferred or required by ASHA guidelines.
• CPR certification preferred.

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders.
• Develop and implement individualized therapy plans with measurable goals.
• Provide therapy to improve communication, articulation, language, voice, fluency, and swallowing.
• Educate patients and families about disorders, therapy techniques, and progress strategies.
• Maintain documentation of evaluations, progress notes, and treatment plans in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.

Position Overview

The Speech Language Pathologist (SLP), is responsible for providing a full range of speech-language pathology services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

• Evaluates patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews.
• Develops and implements individualized therapy plans based on assessment findings, setting measurable goals to address patients' specific needs.
• Provides therapy to improve patients’ communication skills, including articulation, language comprehension, voice modulation, and fluency, as well as swallowing and feeding abilities.
• Educates patients and their families about the nature of disorders, therapy techniques, and strategies to support progress and enhance communication and swallowing in daily life.
• Maintains accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans in compliance with facility policies and regulatory requirements.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses And Certifications

Must have and maintain a current, valid state SLP licensure

Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred or required

Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
