Job summary

Park Ridge Nursing and Rehabilitation Center is seeking a Speech-Language Pathologist to assess referred patients and provide direct rehabilitation services in a long-term care setting in Florida. The role involves designing and implementing individualized treatment plans under physician direction while collaborating with the care team.

Responsibilities include documentation, progress notes, and family education, with emphasis on patient-centered care and compliance with HIPAA and residents'

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Must hold a valid professional license and registration for the state of Florida.
• Experience with long-term care and/or geriatric care is preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to evaluate medical condition and functional status.
• Develop and administer a treatment plan with goals aligned to patient capacity under physician direction.
• Set realistic, achievable goals for patients.
• Document and record patient condition and progress.
• Educate patients and families on therapy methods.
• Conduct evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform duties assigned by the Director of Rehabilitation.
• Maintain confidentiality, HIPAA compliance, and residents' rights.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services


• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.


• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.


• Set realistic and achievable goals for their patients


• Document and record the patients' condition


• Educate patients and families in an appropriate therapy method.


• Evaluations, weekly progress notes, recertification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:
