Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to assess patients and tailor rehabilitation plans tailored to each individual's needs. You will develop and administer treatment programs, document progress, and educate patients and families under physician direction.

Ideal candidates hold NY licensure, have a degree in Speech-Language Pathology, and bring experience in geriatric or long-term care settings with strong documentation skills

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Licensed and registered to practice in NY state.
• Experience with long-term/geriatric care preferred.
• Strong documentation writing skills.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Develop and administer individualized treatment plans with goals.
• Document patient progress, discharge planning, and communications with families.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and comply with HIPAA and residents' rights.

Speech Language Pathologist (SLP)

Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long‑term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Compensation: $55/hour
