Job summary

Topaz Fiscal Services LLC is seeking a Speech Therapist to assess and treat patients in a long-term care setting. The role involves direct patient care, development of treatment plans, and ongoing documentation of patient progress.

You will collaborate with the care team to deliver patient-centered rehabilitation while educating patients and families. Licensure in NY and experience in geriatric care are preferred.

Qualifications

• Graduated from an accredited Speech Therapist college or university.
• Valid professional license and registration for state of NY.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Assess referred patients for rehabilitation needs and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity and physician direction.
• Set realistic goals and document patients' condition and progress.
• Educate patients and families in appropriate therapy methods.
• Perform evaluations, weekly progress notes, recertification and discharge documents.
• Collaborate with all disciplines to provide patient-centered care.
• Complete duties as assigned by the Director of Rehabilitation.
• Maintain confidentiality and comply with HIPAA and residents' rights.

Job description

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
