Job summary

Desert Peak Care Center in Phoenix, AZ seeks a Speech Language Pathologist (SLP) to evaluate, diagnose and treat residents with communication, cognitive-linguistic and swallowing disorders. You will work with an interdisciplinary team to help residents regain independence and improve quality of life.

Responsibilities include evaluations, personalized treatment plans, therapy delivery, and education for residents, families and staff.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Arizona licensure as a Speech-Language Pathologist (or eligibility to obtain).
• CCC-SLP certification preferred; CPR certification encouraged.
• Experience in a skilled nursing facility or with geriatric populations preferred.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for residents with speech, language, cognitive, and swallowing disorders.
• Provide direct therapy services in accordance with the plan of care.
• Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive resident care.
• Educate residents, families, and staff on communication strategies and safe swallowing techniques.
• Document evaluations, treatments, and resident progress in compliance with regulations.
• Participate in care planning meetings, discharge planning, and quality improvement activities as needed.
• Provide training and supervision to Speech-Language Pathology Assistants (if applicable).

Education

Master’s degree in Speech-Language Pathology from an accredited program

Arizona licensure as a Speech-Language Pathologist (or eligibility)

Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred

Current CPR certification

Experience in a skilled nursing facility or geriatric populations preferred
