Responsibilities

• Develop, implement, monitor, and modify individualized treatment plans.
• Provide habilitative and rehabilitative therapy services to residents.
• Assess oral, pharyngeal, and esophageal function related to swallowing disorders.
• Maintain accurate and timely documentation within the electronic health record (EHR).
• Collaborate with physicians, nurses, therapists, dietitians, psychologists, and interdisciplinary team members.
• Participate in care planning meetings and provide clinical recommendations.
• Educate, train, and supervise staff regarding therapy recommendations and treatment protocols.
• Monitor treatment effectiveness and revise therapeutic programs based on resident outcomes.

POSITION OVERVIEW

As the facility's Speech‑Language Pathologist, you will serve as the recognized clinical expert in the assessment, diagnosis, treatment, and management of speech, language, cognitive‑communication, and swallowing disorders.

You will develop individualized treatment plans, collaborate with interdisciplinary teams, provide staff education, and ensure residents receive exceptional therapeutic care while promoting the highest standards of clinical excellence.

PRIMARY RESPONSIBILITIES

• Evaluate, diagnose, and treat speech, language, cognitive‑communication, and swallowing disorders.
• Perform comprehensive speech‑language and dysphagia assessments.
• Develop, implement, monitor, and modify individualized treatment plans.
• Provide habilitative and rehabilitative therapy services to residents.
• Assess oral, pharyngeal, and esophageal function related to swallowing disorders.
• Maintain accurate and timely documentation within the electronic health record (EHR).
