Job summary

Astoria Health & Rehab Center in Ohio seeks a Speech-Language Pathologist to help restore independence in residents within a skilled nursing facility. You will guide patients toward optimal function, collaborate with physicians and therapy teams, and educate on maintaining progress after discharge through personalized exercise programs.

The position requires a Master’s in Speech-Language Pathology and an active license, along with basic computer skills for EHR use.

Qualifications

• Master’s Degree in Speech-Language Pathology and active, unencumbered license.
• Must possess basic computer skills and ability to navigate through software and electronic health records.

Responsibilities

• Return patients to optimal physical function in a SNF setting and facilitate achievement of therapy goals.
• Instruct patients in maintaining function after discharge through education and independent exercise.
• Work within a team to support patient, physician, and therapy goals.

Position Summary:

The primary purpose of your job is to return patients to optimal physical function in a SNF setting. You will facilitate achievement of patient, physician, and therapy goals. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.

Education/Qualifications:

• Employee must hold a Master’s Degree in Speech-Language Pathology from an accredited college or university and hold an active, unencumbered license from the state of practice.

• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.
