Responsibilities include developing individualized therapy plans, collaborating with the healthcare team, documenting progress, and providing patient and family education for improved functional independence.
Evaluate residents for speech, language, cognitive, and swallowing (dysphagia) disorders.
Develop and implement individualized therapy plans to meet residents' specific rehabilitation goals.
Work closely with nurses, occupational therapists, physical therapists, dietitians, and social workers to create a comprehensive and effective care plan.
Maintain thorough records of assessments, treatment plans, progress notes, and discharge summaries.
Educate residents and their families about therapy goals, strategies to maximize quality of life, and appropriate diets for swallowing disorders.
Provide support and education for patients and families facing end-of-life care, focusing on communication and swallowing needs.
A graduate degree in Speech-Language Pathology from an accredited program.
A Certificate of Clinical Competence (CCC).
Strong clinical knowledge and experience in adult and geriatric care.
Empathy, adaptability, and excellent communication and collaboration skills.
