Job summary

Pathways Healthcare in Concord, NH is seeking a Per Diem Speech-Language Pathologist to evaluate home environments, plan care, and deliver therapy in patients' residences. Responsibilities include assessing needs, administering therapy, recommending devices, and coordinating with physicians to ensure integrated care.

Candidates must have a Master’s in Speech-Language Pathology, ASHA CCC, NH and ME licensure, at least one year of clinical experience, strong communication and organizational

Qualifications

  • Master’s degree in Speech-Language Pathology with internship.
  • ASHA CCC-SLP credential required or in process per regulations.
  • Current NH and ME licensure to practice SLP.
  • Minimum of one year clinical experience in speech-language pathology.
  • Strong verbal/written communication and organizational skills.
  • CPR certification and valid driver’s license with reliable vehicle.

Responsibilities

  • Evaluates home environment and identifies equipment needs related to speech-language function.
  • Develops and implements plan of care to improve patient function.
  • Administers speech therapy using specialized techniques/equipment.
  • Recommends communication devices as indicated.
  • Collaborates with referring physicians to ensure coordinated care.
  • Participates in case conferences and organizational meetings as required.
  • Documents notes per policy and maintains clinical competency.
  • Identifies needs for other services and makes referrals as appropriate.
  • Instructs patient and caregivers in treatment regime; may supervise home health aides.
  • Performs additional duties to support delivery of speech-language services.

Position Details:
  • Main Office location: Concord, NH
  • Service Area: Hooksett & surrounding areas
  • Job Type: Per Diem
Responsibilities:
  • Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
  • Identifies functional speech defects and establishes a plan of care to improve patient’s function.
  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommends communication devices/aids as indicated.
  • Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.
  • Prepares and submits notes per organization policy.
  • Maintains clinical competency in speech language pathology practice and theory.
  • Identifies patient and family/caregiver needs for other services and refers as appropriate.
  • Instructs patient, family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.
  • May supervise home health aide in procedures directly related to the speech–language therapy plan of care.
  • Performs other duties as required to facilitate the delivery of speech language pathology services.
Qualifications:
  • Possesses a degree that includes an internship from an approved Master’s program in Speech–Language Pathology, or as required by applicable law and/or regulation.
  • Meets the education and experience requirements for a Certificate of Clinical Competence in Speech–Language Pathology, granted by the American Speech–Language–Hearing Association (ASHA).
  • Is certified to practice Speech–Language Pathology in New Hampshire and Maine.
  • A minimum of one (1) year’s clinical experience in Speech–Language Pathology.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
