Job summary

GEM Transitional Care Center is seeking a full-time Speech Language Pathologist to join our in-house team. You will provide assessment, treatment, and rehabilitation services to residents, collaborating with therapists, patients, and families to guide personalized therapy plans.

The role requires a licensed SLP, CPR certification, and 1–2 years of long-term care experience. A consistent schedule and comprehensive benefits are provided as part of the team.

Qualifications

• CPR certification required.
• Licensed Speech Language Pathologist with current state licensure required.
• 1–2 years’ experience in long-term care preferred.
• Must be able to meet physical requirements including lifting up to 40 pounds.
• Ability to follow oral and written instructions and work under supervision.

Responsibilities

• Provide speech-language pathology assessment, treatment, and rehabilitation services.
• Treat patients following standard of care and practice in Speech-Language Pathology.
• Monitor and document resident treatment responses and progress.
• Communicate observations with staff, physicians, and Rehab Director.
• Follow facility policies and maintain accurate documentation.
