Job summary

Stellar Health Group in Vermont is seeking a compassionate Speech Therapist to join our team and provide high-quality speech therapy services to our patients. You will evaluate, plan, and implement individualized treatment plans within a collaborative care environment.

You will work with patients, families, and healthcare teams, document progress, participate in family conferences, and meet productivity and quality standards. A Vermont state license is required.

Qualifications

• VT license required to practice in Vermont.
• Ability to thrive in a fast-paced clinical environment.
• Excellent communication and interpersonal skills.

• Familiarity with EMR and documentation systems.

Responsibilities

• Provide speech therapy services including evaluation, treatment planning, and implementation of individualized plans.
• Develop and maintain relationships with patients, families, and healthcare teams.
• Collaborate with other disciplines to ensure comprehensive care and optimal outcomes.
• Document patient progress and treatment plans timely and accurately.
• Participate in family conferences and education sessions to promote understanding and engagement.
• Meet productivity and quality standards set by the facility and regulatory agencies.
