The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a Speech Language Pathologist (SLP) to evaluate and treat residents with speech, language, voice and swallowing disorders within a collaborative rehab team. The role emphasizes resident-centered care, interdisciplinary collaboration, and documentation in line with regulatory standards.

Active FL SLP license required; CCC preferred; experience in skilled nursing or rehab setting is desirable.

Qualifications

• Active Florida Speech Language Pathologist (SLP) license in good standing.
• CCC certification (Certificate of Clinical Competence) preferred.
• Previous experience in skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on resident needs and goals.
• Provide speech therapy services to improve communication, cognition, and swallowing function.
• Monitor resident progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support positive resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards.
• Educate residents and families regarding therapy programs and treatment goals.
• Participate in care planning meetings and discharge planning processes.
• Ensure compliance with all federal, state, and facility policies and procedures.
• Maintain a safe, organized, and supportive therapy environment.
