Job summary

Fairview Skilled Rehab & Nursing Home in New York is seeking a licensed Speech-Language Pathologist to join our rehabilitation team. The Speech Therapist will evaluate, diagnose, and treat residents with speech, language, cognitive-communication, voice, and swallowing disorders while helping them achieve the highest level of independence and quality of life.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current New York State SLP license.
• CCC-SLP certification preferred.
• Skilled nursing or rehabilitation experience preferred.
• Strong communication, organizational, and interpersonal skills.
• Compassionate, dependable, and committed to providing exceptional resident care.

Responsibilities

• Evaluate and treat residents with speech, language, cognitive, voice, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Perform bedside swallowing evaluations and recommend diet modifications when indicated.
• Maintain accurate, timely, and compliant clinical documentation.
• Collaborate with physicians, nursing staff, dietitians, and rehabilitation professionals.
• Participate in interdisciplinary care planning and team meetings.
• Educate residents and caregivers on communication and swallowing strategies.
