Job summary

Enhance Rehabilitation is currently seeking a qualified Speech Language Pathologist to join our dynamic team. In this role, you will evaluate and treat patients by following an individualized plan of care following an evaluation.

This position emphasizes delivering patient-centered care in a skilled nursing facility, with strong communication with patients, families and the treatment team. Experience with standardized speech/language assessments, swallowing evaluations, and developing treatment

Qualifications

• Demonstrate the knowledge and skills to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills to communicate with patients, families and the treatment team.
• Experience with administering standardized speech/language assessments, swallow evaluations, developing treatment plans and monitoring progress.

Responsibilities

• Evaluate and treat patients by following an individualized plan of care after evaluation.
• Provide patient-centered care in a Skilled Nursing Facility (SNF) setting.
• Collaborate with patients, families and the treatment team to monitor and adjust treatment plans.
