Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist to provide and direct Speech Therapy services, including assessment, treatment, and program development under physician orders. This on-site position emphasizes patient-centered communication care within a skilled nursing environment.

Responsibilities include screenings, evaluations, plan development, family and team communication, discharge planning, and supervision of students.

Qualifications

• Master’s degree in Speech-Language Pathology and state licensure or eligibility are required.
• Maintains ongoing education and development appropriate for the position.

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals as appropriate.
• Conducts screenings of patients to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implements plans.
• Interprets and communicates evaluation findings to patients, families, and team members.
• Establishes patient schedules in conjunction with team members.
• Develops treatment plans with short and long-term goals and methods.
• Coordinates treatment plan with patient, family and other team members.
• Monitors response to intervention and modifies treatment as needed.
• Recommends equipment and instructs on use of augmentative communication devices.
• Participates in discharge planning and develops home/community programming.
• Documents results and submits statistics; supervises students/volunteers as required.
• Adheres to infection control and OSHA standards; participates in QA programs.
