Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist to provide and direct Speech Therapy services, including assessment, treatment, program development and staff education under physician orders.

The SLP will screen patients, develop and implement individualized plans, coordinate care with families and the care team, document progress, supervise students, and participate in discharge planning. The role is on-site at The Grand Nursing in Queens, NY.

Qualifications

• Masters degree in Speech-Language Pathology.
• Current state licensure or license eligible.

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals when appropriate.
• Conducts screenings of patients at regular intervals to determine need for intervention/treatment.
• Evaluates patients and conducts specialized evaluations as indicated.
• Interprets and communicates evaluation findings to patients, family, and team members.
• Establishes individual patient schedules in conjunction with other team members.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plan with patient, family, and team members.
• Monitors patient progress and modifies treatment as indicated.
• Recommends and obtains equipment to improve patient’s communication; instructs on use of devices.
• Participates in discharge planning.
• Develops home/community programming to maintain improvement.
• Documents results of assessments and treatment; submits statistics.
• Supervises students and volunteers as required.
• Pursues continuing education and professional development.
