Job summary

Sweetwater Care in Fresno, CA is seeking a licensed Speech-Language Pathologist to support patients with communication and swallowing disorders in a SNF setting.

You will evaluate patients, develop treatment plans, deliver therapy, document progress, and collaborate with physicians and the care team to ensure high-quality, compliant care that improves patients' day-to-day communication and safety, while supporting care coordination and family education.

Responsibilities

• Direct all aspects of quality patient care, including evaluation, treatment planning, goal setting, and documentation.
• Provides professional treatment while using therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Demonstrates knowledge of Medicare, Medicaid and other reimbursement systems.
• Develops treatment plans and obtains approval from referring physician.
• Collaborates with other therapists and physicians on patient treatment programs.
• Documents evaluations, progress notes, and discharge summaries.
• Maintains compliance with the Compliance Program and Code of Conduct.
