Job summary

Sweetwater Care in California is hiring a Speech-Language Pathologist to assist patients with communication disorders and swallowing safety. You will assess, diagnose, and treat patients, develop individualized plans, and document progress in medical records.

You'll collaborate with physicians, therapists, and families to ensure coordinated care and positive patient outcomes in a SNF setting.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to company guidelines and standards.
• Provides professional treatment while using therapeutic techniques for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Serves as patient and rehabilitation advocate; participates in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans with short- and long-term goals consistent with physician orders.
• Collaborate with other therapists and physicians on patient care programs; document evaluations and progress in medical records.
• Demonstrate knowledge of Medicare, Medicaid and other reimbursement systems.
• Develop effective treatment plans and obtain approval for services from referring physicians.
• Participate in patient care planning and utilization review meetings.
• Provide services appropriate for SNF settings and administer standardized speech/language assessments and swallowing evaluations.
• Document progress notes and discharge summaries in medical records; ensure delivery of quality rehab services.
• Demonstrate courtesy, compassion and respect to patients and staff; adhere to company Code of Conduct and compliance program.
• Coordinate with facility and other organizations on case management needs.
