Speech-Language Pathologist, PRN, Bronson at Home (CCC Required)

Bronson at Home is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our Home Care team on a PRN basis.
In this role, you will evaluate and treat patients experiencing speech, language, cognitive, communication, and swallowing disorders, helping them achieve greater independence and an improved quality of life.
What You'll Do
- Evaluate speech, language, cognitive, communication, and swallowing disorders using standardized assessments
- Develop individualized treatment plans tailored to each patient's needs and goals
- Provide skilled therapy services in the home setting
- Educate patients, families, and caregivers on treatment strategies and home exercises
- Establish functional baselines and monitor patient progress throughout treatment
- Collaborate with physicians, therapists, nurses, and other healthcare professionals to coordinate care
- Participate in interdisciplinary care planning and team discussions
- Screen for hearing concerns as part of the evaluation process
- Serve as a clinical resource and contribute to program development and process improvement initiatives
- Provide community and professional education in areas of expertise
- Perform video swallow studies if applicable to the practice location
Qualifications Required
- Master's degree in Speech-Language Pathology or Communication Disorders from an accredited program
- Current Michigan Speech-Language Pathology license in good standing
- Certificate of Clinical Competence (CCC) through ASHA
- Current Michigan driver's license
- Reliable transportation available for daily business use
- Proof of current automobile insurance
- Ability to obtain BLS certification within three months of hire
- Strong communication, interpersonal, and organizational skills
- Basic computer and mobile device proficiency
- Current NPI number or willingness to obtain one with Bronson's assistance
Preferred
- Previous clinical experience in Speech-Language Pathology Home care experience

Key Responsibilities
- Clinical Care Evaluate patient functional communication, cognition, and swallowing abilities
- Develop and implement individualized treatment plans
- Deliver evidence-based interventions for speech, language, cognitive, and swallowing disorders
- Establish patient goals and determine appropriate duration of therapy services
- Provide patient, family, and caregiver education
- Participate in interdisciplinary rounds and care planning
- Contribute to department initiatives and quality improvement efforts
- Provide consultation and expertise within the rehabilitation team
Documentation & Administration
- Maintain accurate and timely documentation of evaluations, treatment plans, services provided, and patient outcomes
- Complete electronic medical record documentation within established compliance timelines
- Communicate barriers to timely documentation to leadership
- Participate in committees, task forces, and process improvement activities
- Assist with supervision of students and volunteers as appropriate
- Demonstrate effective time management and organizational skills
Home Care Requirements
- Maintain a valid Michigan driver's license
- Have reliable transportation available for daily business use
- Maintain current automobile insurance coverage
- Successfully complete a motor vehicle record (MVR) review following an offer of employment
- Provide updated proof of insurance as required
Physical Requirements
This position requires frequent standing, walking, reaching, bending, and patient interaction throughout the day. Candidates must be able to lift and carry up to 20 pounds regularly and assist with patient mobility activities as needed.
