Job summary

Pro Rehab is seeking a Speech Language Pathologist to float between Elim Wellspring in Princeton and Milaca Elim Meadows in Milaca, MN. The position is On-Call with a wage of $55/hour, depending on experience. CCC-SLP certification and MN licensure are required.

Mileage reimbursement is provided, with benefits available. Responsibilities include assessing and diagnosing communication disorders, developing and implementing treatment plans, providing therapy, educating patients and families,

Qualifications

• Must be a graduate from an Accredited Speech Therapy Program and have a bachelor’s degree.
• Must possess a current accredited MN Speech Therapist License.
• CCC-SLP certification required.
• Homecare Homebase EMR experience helpful but will train.
• Experience with OASIS preferred or willing to be trained.
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
• Develop care plans and discharge planning strategies for optimal patient recovery.
• Collaborate with healthcare professionals to ensure comprehensive and personalized treatment plans.

Responsibilities

• Assess & Diagnose Communication Disorders: Evaluate patient needs and deficits such as but not limited to: patients with speech, language, voice, fluency, and swallowing difficulties.
• Develop & Implement Treatment Plans: Create personalized therapy programs to improve the functional deficits of the patient.
• Provide Speech & Language Therapy: Conduct exercises and interventions to improve communication and speech abilities.
• Educate & Support Patients & Families: Teach strategies for effective communication and techniques to improve daily life.
• Monitor Progress & Adjust Treatment: Track patient improvements, document progress, and modify therapy plans as needed.
• Collaborate with HealthcareTeams: Work with doctors, nurses, and therapists to ensure comprehensive support for patients.

Pro Rehab , an established Rehab company with over 35 years of successful business is hiring a Speech Language Pathologist. We are looking for a Speech Language Pathologist to float between Elim Wellspring, Princeton, and Milaca Elim Meadows, Milaca !We offer a competitive wage, benefits and mileage reimbursement ($.70 / mile). We are seeking an SLP with current CCC-SLP certification.

Position Type: On-Call

Wage Range: $55 / hour depending on experience.

Speech Language Pathologist Responsibilities:
