Job summary

Aviator Home Health is seeking a Speech Language Pathologist to provide rehabilitation services for patients with speech, language, hearing, oral motor, swallowing and cognitive disorders in a home health setting.

Responsibilities include conducting assessments, developing care plans with the physician, delivering therapeutic interventions, and ensuring proper documentation and discharge planning while adhering to infection control standards.

Qualifications

• Master’s degree or doctorate in Speech-Language Pathology from an accredited program.
• Current Texas State License in Speech-Language Pathology and Texas Driver's License.
• Minimum two years' experience in acute care or rehabilitation; home health preferred.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs and suitability for home care.
• Develop/revise Plan of Care in consultation with physician and care team.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders.
• Evaluate outcomes and plan discharge appropriately.
• Apply infection control and standard precautions in patient care activities.
• Accept assignments consistent with education and competence.
• Supervise speech language assistant and HHA as appropriate.
• Complete documentation in a timely manner.
• Meet continuing education requirements and participate in in-service programs.
• Communicate with family and agency personnel to advise and consult.

Job Summary

A Speech Language Pathologist who provides provides therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.

Essential Functions

• Perform initial and ongoing clinical assessments to identify patient needs, level of functioning, ability to communicate, appropriateness of patient for home care, including OASIS assessments at appropriate time points.
• Develop/revise Plan of Care in consultation with the physician and other care team members.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders with appropriate tests and measurements.
• Evaluate outcomes of care with appropriate discharge planning.
• Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Supervise speech language assistant and HHA as appropriate.
• Complete appropriate documentation in a timely manner.
• Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs.
• Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
• Promote Agency philosophy and administrative policies.
• Demonstrate commitment, professional growth and competency.

Job Qualifications

• Education: Master’s Degree or doctorate in Speech-Language Pathology from an accredited college or university
• Licensure: Current Texas State License in Speech-Language Pathology, current Texas Drivers License
• Experience: Two years experience in an acute care or rehabilitation setting, home health preferred.
