Job summary

Tennessee Quality Care is seeking a PRN Speech Language Pathologist to deliver one-on-one home health services in Davidson County, TN and surrounding areas. You will conduct evaluations, develop plans of care, and provide direct speech therapy within a provider’s plan of care, with travel to patients' homes and occasional on-call duties.

The role requires ASHA CCC-SLP certification, state licensure, CPR certification, and the ability to perform home visits, including travel and documentation.

Qualifications

• ASHA CCC-SLP certification required.
• Licensed Speech Therapist in practicing state.
• CPR certification current.
• Ability to perform home visits.
• Reliable transportation, valid driver’s license, and appropriate auto insurance.

Responsibilities

• Conduct evaluations and assess home environment as appropriate.
• Identify functional speech defects and create a care plan.
• Administer diagnostic speech and language evaluations and informal screenings.
• Recommend communication devices as indicated.
• Prepare diagnostic reports and recommendations for the provider.
• Advise and teach clients, families and staff on procedures and activities.
• Provide direct speech therapy services and document patient responses.

Schedule:

Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

What You’ll Do:

• Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
• Identifies functional speech defects and establishes a plan of care to improve patient’s function.
• Administer diagnostic speech and language evaluations and some basic, informal audio logical assessment in accordance with the plan of care
• Recommends communication devices/aids as indicated.
• Prepare a report of each client’s diagnostic evaluation and make recommendations
• Advise, consult and teach appropriate procedures and supportive activities to clients, family members, and other agency staff
• Provide direct speech service to clients under a provider’s plan of care. Records and reports to the provider the client’s response to therapy

Qualifications:

• Possess certification of clinical competence in speech from the American Speech Language Hearing Association
• Licensed as a speech therapist in the state in which practicing
• At least one (1) year of acute care setting or equivalent experience
• Possess knowledge of principles and practices of speech therapy
• Complies and maintains current CPR certification.
• Ability to make home visits and provide nursing care in the home environment
• Must have reliable transportation, current driver's license, and appropriate automobile insurance
