Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Speech-Language Pathologist (SLP) to support residents with person-centered care in our in-house rehab team.

Must have an active Texas license in Speech-Language Pathology and ASHA Certification; SNF/long-term care experience preferred. The role includes evaluations, treatment planning, AAC, education, and collaboration with nursing and therapy staff.

Qualifications

• Experience in SNF/long-term care settings is preferred.
• Knowledge of current SLP treatment practices.
• Familiarity with Net Health Electronic Documentation System is a plus.
• Ability to work with residents with different personalities.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve independence.
• Provide AAC systems for language disorders.
• Educate residents, caregivers and staff.
• Coordinate with other professionals for diagnostic procedures as ordered.
• Maintain accurate billing records and clinical documentation.
• Ensure confidentiality and compliance with laws.

Job description

We have an amazing opportunity for aPer Diem (PRN) Speech Language Pathologist!

We are looking for aSLPto join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

SLP Responsibilities:

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client’s independence and functioning.
• Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
• Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate and timely billing records and supporting clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other Care staff.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for all patient information.
• Complies with federal, state, and local legal and professional requirements.

SLP Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

SLP Requirements:

• Active/Valid Texas license in Speech-Language Pathology
• Current ASHA Certification
• In good standing with all regulatory agencies and licensing boards
