Job summary

Silver Tree Nursing & Rehabilitation is seeking a Per Diem Speech-Language Pathologist (SLP) to join our in-house rehab team serving residents in a long-term care setting. You will evaluate swallowing and cognitive deficits, develop individualized plans and provide AAC solutions as needed.

The position requires an active Texas SLP license and current ASHA certification, with flexibility for per diem scheduling and the ability to coordinate care with a multidisciplinary team.

Qualifications

• Licensed in Texas as a Speech-Language Pathologist (SLP).
• ASHA Certificate in good standing.
• Experience providing speech therapy in SNF/long-term care settings.
• Familiarity with Net Health EHR documentation.

Responsibilities

• Provide skilled, medically necessary SLP assessments and develop individualized treatment plans.
• Implement therapy to improve cognition and swallowing; use AAC for severe disorders.
• Educate residents, families and caregivers; document outcomes and billing records.
• Collaborate with physicians, nurses and Restorative staff; coordinate procedures as ordered.

We have an amazing opportunity for aPer Diem (PRN) Speech Language Pathologist!

We are looking for aSLPto join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

SLP Responsibilities:

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client’s independence and functioning.
• Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
• Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate and timely billing records and supporting clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other Care staff.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for all patient information.
• Complies with federal, state, and local legal and professional requirements.

SLP Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

SLP Requirements:

• Active/Valid Texas license in Speech-Language Pathology
• Current ASHA Certification
• In good standing with all regulatory agencies and licensing boards
