• Make a difference by providing safe patient care, delivering individualized physician care plans, working as part of an interdisciplinary team, and supporting patients and families.

Location: Co. Springs Branch

Coverage area: El Paso County

Responsibilities:

1. Assess and analyze patient care needs; establish goals with the patient/caregiver; develop a care plan including discharge planning.
2. Evaluate the patient's environment for safety and suitability for care.
3. Assess support systems and use of community resources to ensure optimal care.
4. Identify therapy diagnosis and interventions; update care plans as needed.
5. Interpret and implement physician's orders related to Speech Therapy.
6. Ensure orders are carried out ethically and in compliance with regulations.
7. Coordinate the care plan with other disciplines and document changes.
8. Re-evaluate patient progress; modify therapy plans under physician guidance.
9. Educate patients and caregivers on diagnosis, medications, safety, and infection control.
10. Conduct medication reviews; inform about medication regimens and side effects.
11. Provide educational materials to patients and families.
12. Collaborate with other care providers to improve patient outcomes.
13. Complete all documentation accurately and timely.
14. Ensure compliance with standards and regulations.
15. Manage equipment and supplies to support patient independence.
16. Demonstrate therapeutic skills; coordinate care with internal and external providers.
17. Instruct on safety measures; maintain infection control standards.
18. Recognize patient status changes; report appropriately.
19. Document patient status and care in compliance with standards.
20. Proficiently use OASIS documentation; respond accurately and timely.
21. Report incidents as per standards.
22. Follow policies and procedures; use safe body mechanics.
23. Participate in team meetings and training; maintain confidentiality.
24. Contribute to quality improvement initiatives.
25. Perform other duties within scope of practice as assigned.

Qualifications:

• Master’s or Doctoral Degree in Speech Language Pathology from an accredited school.
• Valid state license.
• Current CPR certification.
• Preferred: prior home care experience.
• Ability to perform manual and non-manual therapy exercises.
• Skill in evaluating cognitive, swallowing, speech, and language functions.
• Professional appearance and demeanor.
• Computer literacy and ability to learn industry software.
• Excellent communication, organizational, and time management skills.
• Ability to work effectively in a team and develop critical thinking skills.
