How you will contribute (not limited to):

Responsibilities:
- Assess and analyze patient care needs; establish goals with the patient/caregiver; develop a care plan including discharge planning.
- Evaluate the patient's environment for safety and suitability for care.
- Assess support systems and use of community resources to ensure optimal care.
- Identify therapy diagnosis and interventions; update care plans as needed.
- Interpret and implement physician's orders related to Speech Therapy.
- Ensure orders are carried out ethically and in compliance with regulations.
- Coordinate the care plan with other disciplines and document changes.
- Re-evaluate patient progress; modify therapy plans under physician guidance.
- Educate patients and caregivers on diagnosis, medications, safety, and infection control.
- Conduct medication reviews; inform about medication regimens and side effects.
- Provide educational materials to patients and families.
- Collaborate with other care providers to improve patient outcomes.
- Complete all documentation accurately and timely.
- Ensure compliance with standards and regulations.
- Manage equipment and supplies to support patient independence.
- Demonstrate therapeutic skills; coordinate care with internal and external providers.
- Instruct on safety measures; maintain infection control standards.
- Recognize patient status changes; report appropriately.
- Document patient status and care in compliance with standards.
- Proficiently use OASIS documentation; respond accurately and timely.
- Report incidents as per standards.
- Follow policies and procedures; use safe body mechanics.
- Participate in team meetings and training; maintain confidentiality.
- Contribute to quality improvement initiatives.
- Perform other duties within scope of practice as assigned.
Qualifications:
- Master’s or Doctoral Degree in Speech Language Pathology from an accredited school.
- Valid state license.
- Current CPR certification.
- Preferred: prior home care experience.
- Ability to perform manual and non-manual therapy exercises.
- Skill in evaluating cognitive, swallowing, speech, and language functions.
- Professional appearance and demeanor.
- Computer literacy and ability to learn industry software.
- Excellent communication, organizational, and time management skills.
- Ability to work effectively in a team and develop critical thinking skills.
