How you will contribute

• Provide safe patient care and develop individualized care plans in collaboration with physicians and the interdisciplinary team.
• Assess patient needs, environment, and support system to optimize care.
• Interpret physician orders and implement speech therapy services accordingly.
• Document care, re-evaluate patient progress, and adjust care plans as needed.
• Educate patients and families on diagnosis, medications, safety, and infection control.
• Coordinate with other care providers and ensure compliance with regulations and documentation standards.
• Manage equipment and supplies, and participate in quality improvement activities.

Qualifications

• Master’s or Doctoral Degree in Speech Language Pathology from an accredited institution.
• Valid state license.
• Current CPR certification.
• Preferred: prior home care experience.
• Strong communication, organizational, and critical thinking skills.
