Alpine Care Home Health is seeking a Speech Language Pathologist for per visit home health services. You will assess and treat patients in their homes, set goals with patients for their care plans, and document progress and discharge planning.

You’ll report patient status to the Clinical Supervisor and referring physician and participate in team meetings and ongoing education. Work includes performing OASIS-C assessments and maintaining competency in clinical skills, Microsoft Office, and care

Formación

• Current unencumbered State Professional License.
• One year home care SLP experience with competence in OASIS-C.
• Ability to make sound professional clinical judgment.

Responsabilidades

Responsibilities

The Speech Language Pathologist Per Visit provides speech therapy services in the provision of personal care to the patient. As a Speech Language Pathologist you will:

• Assess and evaluate patient needs/problems, identifies mutually agreed upon goals with patients for patient care plan
• Provide speech therapy services including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient care plan
• Report patient status to Clinical Supervisor and referring Physician
• Document daily changes and updates to the patient care plans, including discharge planning
• Attend staff meetings, team conferences and educational in‑services per agency requirements
• Perform all OASIS time point assessment per Medicare Criteria and submits recertification paperwork per policy and procedure

Qualifications

• A current unencumbered State Professional License
• Current driver’s license and proof of insurance
• One‑year experience as a home care, professional Speech Language Pathologist, and is competent in performing comprehensive assessment and OASIS‑C
• The ability to make sound professional clinical judgment
• The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs
• Proficiency in clinical skills
• Proficiency in Microsoft Office
