Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist (SLP) to provide and direct Speech Therapy services, including assessments, treatment planning, and execution under physician orders. Responsibilities include screenings, evaluations, communicating findings to patients and families, scheduling, discharge planning, and supervision of students/volunteers.

Licensure or eligibility is required with a focus on teamwork and continued education.

Qualifications

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals.
• Conducts screenings of patients at regular intervals to determine need for intervention/treatment.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and team members.
• Establishes patient schedules in conjunction with team members.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plan with patient, family, and team members.
• Monitors response to intervention and modifies treatment as indicated.
• Recommends and obtains equipment to improve patient communication; trains patient, family and staff.
• Participates in discharge planning.
• Develops home or community programming to maintain enhancements.
• Documents results of assessment, treatment, follow-up, and termination.
• Supervises students and volunteers per state regulations.
• Maintains continuing education and professional development.
• Records indirect and billable times per department procedures.
• Provides treatment per standards and productivity.
• Complies with Infection Control, Universal Precautions and OSHA standards.
• Participates in Quality Assurance programs.
• Adheres to confidentiality standards.
• Performs other duties as assigned.
