Services span the full continuum, delivered in a cost-conscious, quality-focused, customer-oriented manner. Core responsibilities include establishing and evaluating treatment plans with patients and families, implementing therapy, documenting progress, and coordinating discharge and referrals

Qualifications

• Evaluates patients and develops the speech/language plan of care in the areas of speech, language, voice, cognition and swallowing.
• Compliance with all applicable regulatory standards.
• Maintains confidentiality of all patient information.
• Adheres to ethical business practices.

Responsibilities

• Establishes a plan of care in collaboration with the patient and/or family members.
• Implements the speech language therapy treatment plan.
• Documents the evaluation and plan of care according to departmental guidelines.
• Completes discharge instructions, follow-up and referrals as appropriate.
