About Keller Oaks Healthcare Center

Keller Oaks Healthcare Center is a skilled nursing facility offering long-term care, post-acute rehab, and geriatric outpatient therapy services. Join our team of clinicians and enjoy opportunities for training and continuing education in areas such as dysphagia, dementia, stroke, neuro rehabilitation, and respiratory/COVID recovery programs.

Responsibilities

1. Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing care plans that adhere to regulatory and clinical standards.
2. Deliver rehabilitative treatments to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with practice requirements.
3. Evaluate treatment outcomes, modify services as needed, and make referrals.
4. Provide consultation and counseling to patients, families, caregivers, and other providers regarding speech disorders.
5. Generate comprehensive discharge summaries following regulatory and clinical guidelines.

Qualifications

• Speech-Language Pathology license is required.
