Duties:

• Effectively screen and evaluate patients with physical functioning disorders and/or wound care, developing appropriate plans of care following all regulatory and clinical standards.
• Provide rehabilitative, skilled, and medically necessary treatments to patients with physical disorders and/or wound care, in accordance with regulations and clinical practices.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, wound care, etc.
• Offer consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Prepare comprehensive discharge summaries of services provided, complying with regulatory and clinical requirements.

Qualifications:

Licensed Physical Therapist. This position is open to PTs at all experience levels. New graduates are welcome!
