Job summary

Evolve Therapy Services, LLC in Sutherland, NE is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. You will plan, direct, and deliver Speech Language Pathology services aligned with federal, state, and local standards, ensuring top-quality resident care.

Responsibilities include evaluating residents, developing treatment plans with measurable goals, and training staff to carry over therapeutic gains.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate data for treatment planning and implementation.
• Develop treatment plans with short-term and long-term goals.
• Instruct patients, family, and staff to carry over therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to ensure needs are met timely with clinical excellence.
