Job summary

Evolve Therapy Services, a therapy management organization, seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Altoona, PA. This PRN position focuses on planning, directing SLP services, and ensuring high-quality resident care in line with federal and state standards.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC certification and active state licensure required.
• Ongoing continuing education to maintain licensure/certification.
• Experience with CMS or regulatory documentation is a plus.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff to ensure carryover of therapy goals.
• Complete compliant documentation in NetHealth EMR per CMS expectations.
• Oversee FEES/MBS protocols and coordinate site programs.
