Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Montgomery, AL. This PRN role focuses on delivering high-quality communication and swallowing therapy to residents, with emphasis on assessment, treatment planning, and interdisciplinary collaboration.

The ideal candidate holds a Master's degree in Speech Language Pathology, CCC-SLP, and an active state license.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plan with target list, short-term and long-term goals.
• Instruct patient, family and staff to carry over remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in screening protocols to meet resident needs.
