Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to provide therapeutic techniques for rehabilitation in patients' homes. You will develop and evaluate care plans, coordinate with an interdisciplinary team, and ensure high‑quality patient outcomes under physician direction.

Qualifications include a Master’s in Speech Language Pathology, state licensure, CCC‑SLP, and 2 years’ experience in acute or rehab settings (home health preferred). A valid driver’s license is required.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from American Speech Language Hearing Association.
• 2 years’ experience in an acute care or rehabilitation setting, home health preferred.
• Valid driver’s license.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well‑being.
• Collaborates with interdisciplinary team.
