Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to provide therapeutic techniques for rehabilitation of patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders. You will work in patients' residences, coordinating care as part of an interdisciplinary team.

Requirements include a Master’s in SLP, current state license, ASHA CCC, and a valid driver’s license with reliable transportation. 2 years of acute care/rehab/home health experience preferred.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited institution.
• Current state license in Speech Language Pathology.
• ASHA CCC (Certificate of Clinical Competence) required.
• Valid driver’s license and reliable transportation.
• 2 years’ experience in acute care, rehab, or home health preferred.

Responsibilities

• Provides coordinated care to patients in their residence.
• Plans, implements, and evaluates patient care plans to restore or maintain well‑being.
• Collaborates with interdisciplinary team.
